In every chapter each source is given in full the first time it is used. Thereafter an abbreviated title is used.  Weblinks are embedded, where available, in the first full source reference.

In quotes, and in the titles of organizations, the spelling follows the style of the original. Elsewhere it follows Bloomsbury’s house style.
